Chief Warrant Officer 2 Phelps was a member of the 299th Attack Helicopter Company, 7th Special Forces Group. On December 28, 1965, he was the pilot of a Bell Iroquois Utility Helicopter (UH-1D) departing An Khe, South Vietnam on a supply mission. Radio contact was lost. His remains were recovered on April 4, 2009 and identified on June 1, 2009.
